Incident Report Forms

13/10/93 -- Mr Cioffi entered the ward at 10:30am. When it was explained to him that he was outside visiting hours and he was requested to leave. At this point he became verbally threatening stating that he would "come back and get me sometime". Cioffi had previously made numerous threats against me varying from shooting to stabbing and as he has a considerable forensic history the matter was reported to the Greensborough police. They felt that as the incident hadn't taken place in a public place they couldn't do anything but if he returned to the ward they advised us to detain him and call them again. At 12:00 Cioffi appeared once again on the ward and was duly detained. The police were called and eventually removed him from the premises at 16:15 and taking him home.

10/10/93 -- Martin's agitation had been increasing as day progressed. Use of PRN medication, counselling and limit setting ineffective. Martin was abusive++, loud & hostile+++. Reported by a co-patient that Martin had shaped up to a co-patient. All concerned denied incident. When Martin was approached re: report Martin denied it however was agitated, abusive and hostile+++. Time out offered to Martin which he refused in a hostile and aggressive manner. Due to potential (see attached sheet).

5/10/93 -- Noel was on level 2-15 min obs. He was having a coffee in the courtyard when he suddenly started to run and jumped over the fence and ran away across the oval towards nursing admin building.

7/10/93 -- Noel rang the ward at 1:50am this morning -- spoke to RPN Norman MacCleod & said he was going to gas himself. Police informed. When police arrived the gas was on full & wet towels were placed under the door. He became abusive towards the police (as reported by police -- threw a knife at them & was restrained & overpowered by them. Noel reports police hitting him over the nose with a torch. Returned by police at 0515 7/10/93. Bruising around face & laceration to bridge of nose evident.

5/10/93 -- After destroying plaster cast on L leg for # leg, patient was taken to PANCH for XR and re-cast leg. Given PRN prior to departure. Patient was promptly attended to by PANCH staff. During waiting for ambulance to return to South 5 Patient became agitated and aggressive. Attempting to throw self off bed at PANCH and attempting to physically assault staff. Whilst being restrained patient attempted to bite staff on R elbow. Patient required I/V medication.

12/10/93 -- Jane climbed upon railing overlooking stairwell at about 8pm. Was threatening to Jump. Staff intervened and coaxed her to halt. Mattress placed on steps. Despite staffs advice to climb down client jumped. No injury. Incident occurred at West End.

13/10/93 -- Patient Jane Donnell, 39 y.o., diagnosis -- Borderline Personality disorder in Crisis, Depression. Found by co-patient Ruth Cabot, sitting on stair-railing threatening to jump, as soon as staff approached Jane attempted to jump, although broken by staff, landing (4) four feet down on steps.
Patient seen by Consultant Psychiatrist in morning, angry about being discharged to c/o private psych, attempting to sabotage discharge plans.

18/10/94 -- SLIPPING ON WET FLOOR

Patient was incontinent on bedroom floor and whilst attempting to go to the toilet slipped in the wet area.

17/10/94 -- INJURY OF PATIENT

Client had absconded from ward yesterday, he was returned by Mill Park Police Officers. Today it was noticed that he has a swollen R eye. He denies any injury or accident, he believes it it related to sunburn!

16/10/94 -- PHYSICAL ASSAULT NOT REQUIRING MEDICAL ATTENTION

Patient was being demanding towards staff re demanding she be given all her cigarettes. When it was explained that staff member would need to find keys and that she may need to wait, she hit staff member on face.

14/10/94 -- ALTERCATION BETWEEN 2 PATIENTS

2 patients seen to be fighting in upstairs corridor. Patients separated by staff and placed in respective rooms (psychiatric emergency called and cancelled.) Duty M.O. called and tended to both clients.

14/10/94 -- PATIENT SELF HARM

Patient jumped fence and reported lacerated R wrist with broken coca-cola bottle. Claims to be sexually harassed by fellow patient.

14/10/94 -- STAFF ACCIDENT

Whilst entering staff lounge; tripped over low coffee table

23/9/94 -- CO-PATIENT INAPPROPRIATE SEXUAL ADVANCEMENT

Patient No. 1 approached staff stating that patient No. 2 had unexpectedly and inappropriately opened his dressing gown and displayed his genitals to her.

21/9/94 -- ASSAULT OF STAFF MEMBER

Dan approached office requesting to be taken to PANCH. Had previously been preoccupied re his breathing. It was conveyed to Dan he would be reviewed by a M.O. as soon as possible. Dan reacted by attempting to physically assault staff member. Psychiatric emergency called. Dan was restrained and secluded.
Previous to Dan's seclusion he was counselled re assaulting staff and harming others. He did not respond to this intervention, continuing to hit out at staff. During the physical altercation Dan sustained a cut R eye. Also a staff member sustained a cut nose. Another staff member sustained ? # ribs, in the ensuing melee.

18/9/94 -- ? RESPONSIBILITY TO RETURN ABSCONDED PTS

Greensborough police notified of absconding patient by ward. Greensborough police notified Northcote police station as patient resided in Northcote area.
Northcote police flatly refused to attend to the situation, stating that it is not their responsibility unless the patient is violent, and stating that it was the responsibility of the ambulance service.

18/9/94 -- THREATENING PROPERTY DAMAGE (Bomb threat)

Answered on incoming call, caller stated "There's a bomb in the building, There's a bomb in the building" and hung the phone up. Unit Manager informed and operational support notified. Evacuation procedure implemented. OPS Support contacted. Evacuated to South 4 court yard. Chief engineer and police attended and gave clearance to re-enter the ward. Re-entered at 1300 -- patients reassured and counselled re incident.

17/9/94 -- SELF HARM BY PATIENT

Helen came to me at 2200hrs to say she had slashed her left arm. On examination multiple superficial traversed lacerations on anterior aspect. Dr informed and applied 2 stitches and the rest was steri-stripped and dressed. Patient counselled. (Razor blade removed & patient & room searched.)

16/9/94 -- PROPERTY DAMAGE

Patient punched and kicked multiple holes in plasterwork in bedroom No 12 and a hole in Doctors Rm No. 1

11/10/94 -- AGGRESSION & PHYSICAL ASSAULT TO STAFF

Became physically aggressive to staff when requested to move out of nursing office door. Prior to this, Erica shouted abuse to allied staff without any provocation. Was offered ventilation & time out without success. Extra medication was being organised when physical aggression occurred.

8/10/94 -- PATIENT ATTEMPTED TO SELF HARM -- NOT REQUIRING MEDICAL ATTENTION

Patient attempted to cut R wrist with a broken tea cup. Co-patient Trevor reported this to staff after seeing patient in upstairs bedroom. Superficial laceration obtained to R wrist.

7/10/94 -- SUBSTANCE ABUSE

Patient Trevor handed to registrar Dr Robert Mayne a small plastic coin-bag with what appeared to be a green leafy substance -- marijuana. Patient handed this to doctor in exchange for weekend leave. This was not granted. Dr Mayne informed nursing staff and this was taken from patient & placed in locked DD cupboard -- to be returned to pharmacy. Patient was counselled and searched for further illegal substance.

4/10/94 -- PATIENT ATTEMPTED TO SELF HARM

Patient scratched recently healed slash wounds on left arm, causing slight bleeding from three (3) scars.

4/10/94 -- PATIENT SELF HARM

Pt demanded to be released when she was refused this. She went to the bathroom where she had previously tied a radio cord around shower-rail. Staff followed her down to bathroom. She had wrapped cord around her neck and when staff entered she dropped on the cord.

1/10/94 -- DAMAGE TO CAR BY PATIENT

Patient picked up rock & threw it & hit my car causing two scratches on back passenger side window plus dent and damage to duco on back passenger side door.

28/9/94 -- ASSAULT TO STAFF BY PATIENT

As per ongoing arrangement I saw Eduardo to talk about the implications of his actions regarding his children. We went to I/V room 2, he sat down, I closed the door & went to sit down and he put his hands up & attempted to put them around my neck. I grabbed his hands & yelled for help. He did not use a lot of force and he did not attempt to knock me out. He just wanted to strangle me. A nurse, Ray came in & restrained Eduardo.

30/12/94 -- DISRESPECTFUL USE OF RECYCABLE BINS

Between the hours of 12 & 2pm, the above named nurse was seen to be hovering over the green recycable bin with a piece of scot towell in her grasp. Seconds later, this towelling paper was seen to be falling from the grasp of the above named offender, into the recycable bin. The offender was then observed to make the overt act of smiling, & rubbing her hands together as she decamped from the scene.
